OBTAINING COVID-19 TESTS & SUPPLIES
If you are unable to secure testing supplies through a commercial provider (preferred strategy), you may request testing supplies from the State of Alaska Emergency Operations Center using a Form 213, available at: https://www.ready.alaska.gov/SEOC/Documents/ICS_213_RR_-_pdf_fillable_form.pdf Please note that if the request has to do with supporting the fishing industry, that information should be included on the 213.

For storage and transport:
- Store all specimens in your refrigerator (2-8°C) up to 72 hours, or freeze for longer storage.
- Pack refrigerated specimens on ice packs to preserve viral integrity. Pack frozen specimens with plenty of ice packs or dry ice.
- Ship as a Biological Substance Category B UN3373. If using dry ice, indicate UN1845 as well.
Each sample needs to be accompanied by a SARS-CoV-2 (COVID-19) Requisition Form (unless an electronic option becomes available). Alaska Medicaid—Pharmacist Billing for COVID-19 Sample Collection Services
Alaska Medicaid has expanded the billable services that can be provided by pharmacists during the COVID-19 public health emergency. The first instructor-led training webinar addressing Pharmacist billing for COVID-sample collection services was held today, June 26th.
